We are temporarily allowing late claims to be submitted and will request the Court to consider these late claims

You have three choices for filing a claim form:

  • First, you can file your claim online with your phone or computer by clicking: ​

  • Second, you can also request that a claim form be sent by email to you by clicking: numireleasecard@sylaw.com. Send that email and we will respond to your email with a claim form that you can mail to us or take a picture of the completed form with your phone and email to us.

  • Third, Click on the image below to retrieve a PDF claim form that you can print and return by mail. You can also take a picture of the completed form with your phone and send by email to: numireleasecard@sylaw.com
